This is a free roundtable session for OCHN and OCHCH members.
With the last quarter of 2024 quickly approaching, we are all working hard to perfect or improve our HH-VBP processes to ensure we are successful in Outcomes, Acute Care Utilization, and HH-CAHPs scores.
As we enter the flu & cold season, and what has historically been a busy season for home health patients in our region, we would like to invite you all to a roundtable discussion about what your agency is doing, has done, would like to do, or is looking for suggestions to improve Acute Care Utilization metrics.
