Join OCHCH and The HAP Foundation for insights on developing a palliative care program.

Many hospices are now offering palliative care as a line of business and there is strong empirical evidence that community based palliative care improves quality outcomes and patient/caregiver experience while also reducing cost.

There is increasing interest among payors on this type of care and currently several states are in various stages of developing state funded palliative care benefits.

In this session, we will talk through the steps a hospice needs to consider when thinking about implementing community based palliative care program as a service offering.

Topics covered will include completion of a community needs assessment, staffing models, options for program design, overview of payment models, insurance contracting and care delivery pathways.  Models from successful programs will be explored and reviewed.
