Please complete this application for membership in its entirety. 

A primary provider is a business or corporation/entity that provides direct health care services in Ohio under common ownership, control, or board direction, including locations, sub-units, and programs providing in-home patient services, hospice, and palliative care.

Annual dues for all provider/voting members are based on gross revenue for the previous fiscal year generated in from all locations and all patient revenue payers related to home and community-based services.

Provider Membership is open to any business that provides home health, hospice, palliative care, case management, therapy, homemaker, and/or home health aide services in Ohio. This is a voting membership with each provider member eligible to cast one vote on important decisions and leadership roles of the association. Annual dues for provider members are based on gross revenue generated in Ohio from all locations and all patient revenue payers.

The OCHCH Education Pass enables no-cost or deeply discounted registration to a wide range of OCHCH-managed and select partner-managed educational opportunities posted on our Education Calendar. An active Council membership is required to purchase an Education Pass. Home Health Aide Live Training Sessions are not included.

Education Pass tiers are based on Annual Gross Revenue

Tier 1, $1,500  |  Revenue under $8M

Tier 2, $2,000  |  Revenue above $8M

The Home Health Aide Training Program Pass is priced at $2,000 annually per member organization. This pass covers:

  • Ten (10) trainee registrations per year.
  • Additional registrations at preferred pricing of $175 each.
