Audits of care programs have become commonplace for Medicare Home Health Providers, and most audits begin with documentation requests for review.

Often, the success of the audit is based on the content of documentation of skilled and qualified care delivery. Lacking a “Documentation for Coverage” model, many Home Health Provider audits lead to denials based on lack of supporting documentation for the claim.
